We are seeking a Field Nurse to join our team and provide home health care services to our patients. The ideal candidate will have experience in outpatient care, as well as a strong understanding of medical terminology and protocols. The Field Nurse will be responsible for providing assessments, treatments, and patient education in the home setting, as well as coordinating care with other healthcare providers. This is an excellent opportunity for a nurse who is looking to make a difference in the lives of their patients. We are looking for candidates that are okay with driving 20 miles with mileage reimbursement.

Responsibilities:

• Provide home health care services to patients in their homes.
• Provide patient education and support.
• Conduct assessments and evaluations of patients' conditions.
• Provide direct nursing care to patients in the home setting.
• Develop and implement individualized care plans for patients.
• Manage medications and treatments prescribed by physicians.
• Monitor patient progress and adjust care plans as needed.
• Maintain patient records and document all activities.

Pay: $45.00 - $50.00 per hour

Expected hours: 10 – 20 per week
